With that colossal of an enclosure, the t could have climbed to the top and fell, either on the cork or the water dish, and ruptured the abdomen or at least got some internal damage.
Sorry for you loss
If you ever think of getting more ts, I would research more on their specific needs, and keep it in a waaaaaaay smaller enclosure.
